POARTA CĂTRE O BĂTRANEȚE DE VIS!

În luna iulie 2019, ASOCIAȚIA GAL POARTA TRANSILVANIEI cu sediul în Negreni, str.Principala nr. 63 D, județul Cluj, cod postal 407440, a început implementarea proiectului POARTA CĂTRE O BATRANEȚE DE VIS!

 

Proiectul este finanţat în cadrul apelului POCU/436/4/4/ Reducerea numărului de persoane aparținând grupurilor vulnerabile prin furnizarea unor servicii sociale/ medicale/ socioprofesionale/

de formare profesională adecvate nevoilor specifice în vederea integrarii socio-profesionale.

                      

Obiectivul general al proiectului Reducerea numărului de persoane vârstnice aflate in situații de dependență și/sau în risc de excluziune socială prin furnizarea de servicii sociale și social-medicale către 162 de persoane vârstnice. Prin activitățile destinate persoanelor din grupul țintă (A 2 Furnizarea serviciilor sociale integrate pentru persoane vârstnice : servicii de ingrijire la domiciliu care vor consta în activități de îngrijire de bază și instrumentală acordate în sistem integrat cu servicii medicale și servicii socio medicale prin Centrul de zi pentru persoane vârstnice unde se vor desfașura activități de socializare tip club și servicii de monitorizare a stării de sanatate) proiectul iși propune să reducă cu un numar de 91 persoane, numărul persoanelor vârstnice aflate în situații de dependență și /sau în risc de excluziune socială.

 

Obiectivele specifice ale proiectului urmăresc Înființarea unui Centru de zi pentru persoane vârstnice și a Serviciului de îngrijire la domiciliu pentru persoane vârstnice.

OS1 este corelat cu Activitatea 1 Dezvoltarea serviciilor sociale integrate destinate persoanelor vârstnice. Rezultat 1: Rezultat 1: A.1 – 1 Centru de zi pentru persoane vârstnice licențiat (S), 1 Serviciu de îngrijire la domiciliu pentru persoane vârstnice licențiate (S), 1 Serviciu de ingrijire la domiciliu pentru persoane vârstnice licențiate (P). Înființarea acestor servicii este primul pas necesar, contribuind indirect la atingerea indicatorilor 4S47 si 4S42, a obiectivului proiectului și a Obiectivului Specific 4.4. al programului. Rezultatul 1 este aferent Activității 1, activitatea care pregatește terenul pentru activitatea relevantă și obligatorie 2 Furnizarea serviciilor sociale integrate destinate persoanelor vârstnice, fiind o condiție obligatorie pentru ca persoanele vârstnice din comunitate să poată beneficia de activitățile proiectului, creându-se astfel premisele ca vârstnicii să depășească situațiile de dependență sau pe cele de risc de excluziune. OS 2. Furnizarea serviciilor sociale integrate destinate persoanelor vârstnice. Rezultat 2: 162 persoane din GT beneficiare ale serviciilor socio medicale (diagnosticare, consiliere psihologică), 162 de persoane din GT beneficiare ale serviciilor din centrul de zi pentru persoane vârstnice, 162 persoane din GT beneficiare de îngrijire la domiciliu (igiena corporală, îmbracare și dezbracare, igiena eliminarilor, hrănire și hidratare, transfer și mobilizare, deplasare în interior, comunicare; ajutor pentru prepararea hranei/livrarea acesteia, efectuarea de cumpăraturi, medicamente, administrare de medicamente, activități de menaj, însoțirea în mijloacele de transport, activități de administrare și gestionare, telefonul vârstnicului).

 

Rezultate așteptate: Rezultat 1: A.1 – 1 Centru de zi pentru persoane vârstnice licențiat (S), 1 Serviciu de îngrijire la domiciliu pentru persoane vârstnice licențiate (S), 1 Serviciu de îngrijire la domiciliu pentru persoane vârstnice licențiate (P). Înființarea acestor servicii este primul pas necesar.

Rezultat 2: A.2.162 persoane din GT beneficiare ale serviciilor integrate socio medicale prin centru de zi pentru persoane vârstnice (diagnosticare, consiliere psihologică).
